A long term physical health condition is one that can't currently be cured but can be controlled with the use of medication and/or other therapies. We are specifically seeking views on self-management of the following conditions:

-      Diabetes

-      Cancer within the last 5 years

-      Heart disease

-      Respiratory conditions (e.g. COPD, Emphysema, Asthma, etc.)

This short survey aims to highlight some of the key issues, challenges and opportunities to supporting people with long term conditions to manage their condition and live well, and will be used to improve services in Cornwall.

Who is this survey for?

This survey is aimed at health, social care or voluntary sector professionals who plan, manage or deliver services for people with long term conditions – particularly the conditions listed above. There is a separate survey for patients and carers.

Who is carrying out the survey?

The survey is being carried out by the Public Health team at Cornwall Council. We are working in partnership with Kernow Clinical Commissioning Group (who plan and pay for healthcare services), healthcare professionals and a number of voluntary sector organisations to improve self management support for people with long term conditions.
